School Health and Information

 

Hi.  My name is Rebecca Clark, RN, BSN.  I am your child’s school nurse.  I would be happy to help you with any health related questions you may have concerning your child.  Please feel free to contact me at any time with any concerns or if I may be of any assistance to you.  Below you will find my schedule, Flexibility is one of the first things we learned in nursing school.  Please be aware that this schedule is subject to change.  You may try the numbers below or you may contact Munday Administration Office at 940-422-4241, option 1, if you are trying to locate me.  You may also e-mail me at rebecca.clark@esc9.net.  I look forward to working with you and your child.  Below my schedule you will find information regarding school health. School Nurse Schedule

When to Stay Home:

Temperature of 99.4 or higher during the past 24 hours.

Vomiting or diarrhea during the night or before school.

Contagious diseases such as chickenpox, strep throat, pink eye, or any unexplained rash.

Students may return to school after 24 hours of antibiotic therapy for strep throat or pink eye.

Medications:

Prescription Medication:  These must be in the original container with the original pharmacy label that has complete instructions including the name of the medication, dosage and times to be given.  The physician’s name must be on the label.  There must also be a written note from the parent or guardian with signature. In this note, a list of any allergies the student has or No Known Drug Allergies if the student does not have any must be included.

Over-the-Counter Medication:  There must be a written note giving permission before any over-the counter-medication can be given.  This must include the name of the student, name of the medication, the dosage, intervals or time and instructions.  This must also have a parent’s signature.  This form can be kept in the office for the school year but expires at the end of each year.

Inhalers:  All inhalers must stay in the secretary’s office unless we have a note from the student’s physician stating that the child is capable of self administration. 

All medication should be taken to the secretary at the beginning of the school day.  If the medication is to be taken home at the end of the school day, it will only be released to the student’s parent/guardian or to the student that brought it.

Immunization Information

All students must be fully immunized against certain diseases or must present a statement for medical reasons or a certificate from the state for reasons or conscience.

For exemptions based on reasons of conscience, the District can honor only official forms issued by the Texas Department of Health.

When your child receives an immunization please bring a copy of the shot record to school so we can keep your child's health record up-to-date.

*A parent/guardian must accompany the child and you must have your child’s immunization record in order to receive a vaccination.  Schedules are subject to change so please call the clinic of your choice and confirm your appointment prior to taking your child out of school or driving to the clinic.

*The influenza (flu) vaccine can also be received at either the Knox City Hospital Clinic or the Munday Clinic.  Call for availability.  

SCHOOL-HEALTH Emergency Rules from DSHS on Vaccinations
The Department of State Health Services will be issuing an emergency rule effective today August 24, 2009, that will permit schools to provisionally enroll students that may not have had certain required immunizations. Under the emergency rule, students who are required to have, but have not yet received one or more of these three vaccinations may be provisionally enrolled in school through September 30, 2009. A student is not required to be vaccinated according to the schedule for the three vaccines until October 1, 2009.  The schedule for one or more of the vaccines may be delayed beyond October 1, 2009, for good cause determined by the commissioner of the department.

This emergency rule will delay the following requirements relating to meningococcal, tetanus/diphtheria/pertussis, and varicella vaccines:

1)      meningococcal vaccine;  

2)      tetanus/diptheria/pertussis (Tdap) vaccines for Texas students entering 7th grade and boosters for students entering grades 8-12; and

3)      varicella vaccine for students entering kindergarten and those entering 7th grade.
